How to make a Police Report 


To report a FORGERY offense, mail or bring the following information to our office: 

A completed “Merchants Form” - This form provides us with the information needed to generate an offense.  It MUST be filled out completely and accurately.  Please pay close attention to the boxes marked with an *.  Please refer to the Merchants Form on this web site for a copy. 

The original forged document(s). 

An “Affidavit of Forgery” - This is a notarized sworn statement attesting that the signature which appears on the questioned document is indeed a forgery and not authorized by the account holder.  This document is needed before we can file criminal charges in ALL cases.  For a copy, please refer to Affidavit of Forgery on this web site.

You MUST be the monetary loser to make this report.  Include any other information you believe may help us solve this crime.

To report a CREDIT CARD ABUSE offense, mail or bring the following information to our office: 

A completed “Merchants Form” - This form provides us with the information
needed to generate an offense.  It MUST be filled out completely and accurately.  Please pay close attention to the boxes marked with an *.  Please refer to the Merchants Form on this web site for a copy. 

The original charge receipt. 

Any other information you believe may help us solve this crime.  You MUST be the monetary loser to make this report.

To report a THEFT OF IDENTITY offense, mail or bring the following information to our office: 

After receiving our Identity Theft Packet, complete the “Identity Theft Form” – This form provides us the information we need to make an offense.  It must be filled out with as much information as possible.

Your identity must be compromised within the Dallas city limits. 

You must provide some sort of documentation that supports your claim.

After we begin our investigation into your offense, you may be required to
report to our office to provide us with further information – for example, a signed affidavit or a thumbprint.  The detective assigned to your case will determine what further information is needed from you.
